Functional Behavior Assessment For Pre-K and School-Age Individuals

This is a comprehensive assessment used to identify the function or purpose of a behavior. For purposes of this assessment the clinical abbreviation “BX” may be
used interchangeably with the word “Behavior” for data reporting. It involves direct observation, interviews/anecdotal notes, and data collection processes commonly
used in applied behavioral analysis services. It is best to include accounts of the student's behavior(s) of concern from multiple sources within multiple settings.
Behavior tracking forms and check-ins should be provided so that revisions to the behavior intervention plan drafted from this assessment can be made as quickly as
possible.
